OWNER HEALTH CERTIFICATION and CONTRACT

This is a contract between Merry Mutts, Inc. (hereinafter called the “Company”) and the pet owner whose signature appears below (hereinafter called “Parent”).

1. The Parent specifically represents that he or she is the sole owner of the pet.

2. The Parent certifies that all information given about said pet is accurate.

3. The Parent specifically represents to the Company that the pet has not been exposed to rabies, distemper or canine cough within a thirty-day period prior to boarding/daycare services.

4. The Parent certifies said pet has been treated for fleas and ticks within a thirty-day period prior to boarding/daycare services.

5. The Parent certifies that said pet has not harmed or shown aggressive or threatening behavior towards any person or any other dog

6. The Parent agrees to be solely responsible for any and all acts or behavior of said Pet(s) while the Pet(s) is in the care of the Company.

7. The Company shall exercise reasonable care for the pet(s) delivered by the Parent to the Company for boarding, daycare, grooming, or other services. The Parent agrees that despite the efforts of Merry Mutts, Inc. staff to provide a safe environment, there may be unpredictable or unforeseeable circumstances that may expose their Pet(s) to health risks or injury. The Parent hereby waives and releases Merry Mutts, Inc., its employees, owners and agents from any claims while on the property.

8. The Parent agrees to pay all costs and charges for services he/she requests. All charges incurred by the Parent shall be payable at the time the Pet(s) is discharged from the Company. The Parent agrees that in the event the charges are not paid in accordance with this contract and proper notice is given, the Company may exercise its rights regarding collection.

9. If the Pet(s) becomes ill, or the state of health or well being otherwise requires professional attention, the Company, at its sole discretion, may engage the services of a veterinarian or administer medicine or other requisite attention to the animal, and it is agreed that all costs and charges thereof shall be paid according to the Merry Mutts Warranty Program up to a maximum of $500.

10. The Company, at its sole discretion, may require a deposit before agreeing to provide services. The Parent agrees to pay such deposit, as required.

11. This contract contains the entire agreement between parties. All terms and conditions of this contract shall be binding on heirs, administrators, personal representatives and assignees of the Parent and the Company.

12. This contract is binding for the present and all subsequent engagements for services of the Company by the Owner.

I hereby acknowledge that I have read all of the above and agree as the Parent of the animal(s) foregoing.
